2. This plan is valid for a standard list price of treatments only.
3. In any ‘Dental Health Plan’ & ‘Comprehensive Health Plan’ you’ll get a coupon for Rs.1500/-, that can be used for any dental treatment.
4. In ‘Super Speciality Dental Health Plan’ you’ll also get discount of Rs.2500/-, that will be apply on treatment cost of a Dental Implant & Orthodontic Braces.
5. In ‘Ultra Premium Dental Health Plan’ & ‘Diamond Health Plan’ you’ll receive additional coupon for Rs.500/-, that can be used for any dental treatment.
6. Only one coupon can be used per bill, the second coupon can be redeemed only after 28 days of first coupon’s full amount being exhausted.
7. The coupon can not be clubbed with other offers or packages.
8. The plan is active till six months from date of purchase.
